Fallout 4 freezes and crashes constantly

Hello, I’m new to the community and I do enjoy the mod, but I’ve been having problems with it freezing and crashing constantly.
When I first downloaded the mod and a few different mods it worked fine for couple of hours in a new game, but it started to act up when I get close to settlements I’ve built. At first I thought it was the other mods I downloaded messing with SS2. So I deleted every mod, re-downloaded SS2 and started a new game. I found out that City Plans was the issue. After I used City Plans on Sanctuary that’s when the problem starts. I’ve used City plans on other settlements, but still have the same problems.
Please help
 
Which xbox version are you on? I know One S is awful for this since I'm on it. And city plans... pretty much impossible to get to level 3 without what you're describing. Also it helps to really take your time, wait for stuff to load, update, etc and not rushing to open workshop or pipboy before that happens.
 
This might be a "triangle of death" problem.

Short, broad-brush version: the settlements of Sanctuary, Red Rocket, and Abernathy Farm are so close together they can choke the engine if too much is happening in them.

Solution: If you don't have access to engine-buffing mods (esp. Baka Script Cleanup), your best bet is to scrape Red Rocket clean, let the Abernathy family live small, and ONLY build up Sanctuary. Then, don't fast-travel to Abernathy or Red Rocket. Fast-travel to Sanctuary, let the engine grind all the scripts for that area, and THEN go to whatever your north-of-map destination was.

My less-brilliant summary:
Fallout 4's script engine (where "script" means "list of instructions for making things move around as if alive") has three key weaknesses...

1) It only runs between GPU renders, and anything it can't finish in a 60th of a second must be paged to RAM
2) The page file size is too small. (on the PC this can be fixed with a mod)
3) It runs all scripts it finds within a certain range of the player. Any in-game location where that range will include multiple settlements can crash the game if those settlements are heavily populated. The "Triangle of Death" includes THREE settlements, and because of where the game starts, players are basically encouraged to turn that corner of the map into a zone which will crash the game.
