What causes my faction to turn on me?

After taking an outpost my faction (Minutemen) start shooting at me and I have to load prior save and retake a few times.

Is it caused by a mod conflict, something I'm doing wrong, or the faction pack (Minutemen)?

Are you taking the outpost for the MM or are you taking it for Jammer and then returning to the MM afterwards and then they are hostile to you ?

Taking it for the Minutemen.
Jammer is dead.
