Yes to FH and NW, but you have to unlock that ability. The FH settlements will work independently of the commonwealth in the meantime.

It should. It's only the quests that are hardcoded to the Commonwealth, the actual gameplay mechanics like building plots and stuff should work anywhere.
To get Caravans etc to 'link' different 'maps' together, though, you need to do a special research thing at the HQ.
