After playing for a couple of weeks, here are my thoughts about both working together. Spoilers will be marked and hidden for those of you who haven't played through already.
First off I've not had any issues with them working together save for a few main quests and the Stranger's clothing. Horizon's items tend to be duplicates of what SS2 can do, so be mindful of what you're wanting to use in your settlements. I tended to use Horizon's items first then replaced them with SS2 as they became available.
The best playthrough I've found is to leave Sanctuary as a vanilla settlement and just use it for the quests. The Stranger tended to get hung up a few times when there was more going on that what was originally planned. Getting the Quincy 5 doesn't matter as SS2 will adjust to whether you got them first or last concerning Jake. Other settlements are fine to assign a city plan as soon as possible. I tended to avoid the settlements until needed, as I tried to get through the main SS2 quests first.

Some caveats:
- Horizon has some very strict requirements of what it will play nice with. Load order is especially important with this mod so please check its wiki page for details.
- SS2: Check the city plans for each to see what requirements are. I personally have gaps in my settlements because I haven't bought Creation Club content. Some city plans are huge and will need for you to expand the settlement build. Use with caution though.
- Other mods: check load order and/or requirements. Make sure they don't interfere with Horizon. As far as I can tell, requirements for the city plans in SS2 aren't deal breakers for Horizon.
For those of you who wish to add Start Me Up:
Dialog from SS2 assumes you're playing a vanilla game. Also if you don't play as the vault dweller, triggering Jake first before entering the vault will cause it not to open when you finally do get to explore it. A non-console fix is to explore the vault before Jake.
