Experiencing the armory bug myself. Though in this case, it just removed the clothes & left the armour. I have the 'automatically equip outfits' turned off in MCM, and this outpost doesn't have an Armory at all! I think I had 21 people here when this screenshot was snapped.
It was. There had just been an unscripted attack in that area--there's a bug spawn to the north & they keep wandering over. So of course everyone goes nuts when this happens. Doing clean up, I heard Maul and Ms. Nanny speaking, so I did miss a bit of it.

This has happened the past 4 or 5 times I've restarted. I've not seen the end of "Burdern to Bear" or any of the Conqueror quest line that follows. And I'm all but certain it's because of broken scenes--or I'll rephrase, interrupted scenes.

Best not to go inside the Saugus Iron Works, or into Dunwich Borers until the quest starts, likewise for Outpost Olivia if you plan on doing Dakka's quests. I know it can be frustrating, but see if you can roll back to an earlier save.
Conquer another settlement. It'll then start the next time you come back to your HQ.

"Unwelcome Guests" should have started when you claimed your seventh settlement.
@Rites actually missed that scene yesterday; it started but was interrupted by a random raider attack. Entire outpost goes nuts, the collared slave was left in her 'sweeping' animation until I left.
Can confirm this also happens to me. Sometimes the flags are already down when I arrive, but more often it's shortly after, but ALWAYS when I arrive at a vassal. The code seems to take a bit of time to catch it & make sure people are assigned again.
I've triggered the Prebuild system outside the vault in the past. The danger is going near any settlement you've selected to prebuild, right? Can result in things not initializing properly, shit like that. I only attempted it a couple of times, back in the earlier days of SS-C.
