Will some communities build hospitals as well as clinics? It would be a nice option. To see doctors actually in action putting an NCP onto a gurney taking them to a clinic or hospital. Just another random thought. Good or horrible idea ?
 
The "settlements build themselves" mechanic is not as complex or intelligent as that; it's basically just a way to 'copy paste' the way another player built that settlement. It just gets described as being the settlers building things so you can 'roleplay' with it.
All the "city plans" (what the pre-designed settlements are called) that exist so far were made only using Chapter 1's stuff. The new stuff will only appear in settlements if either you build it yourself, or people start making more "city plans" specifically for chapter 2.

As a settlement gets established and the NCP's all get jobs I could see all the buildings upgrade over time. One wonders how long the player must avoid the upgrading settlement as it morphs into the upgraded state or would you have to avoid it temporarily at all?

The buildings do upgrade over time, as can the "City" itself if it's one that is set up to do that. The timeframe for that is measured in ingame days at the shortest, it doesn't actually care whether or not you're there if you set those settings up that way.
 
speaking on SS2 doctor, they stop healing player when activated Survival mode, is that intended?
No it isn't, I've seen that happen too. The shopkeepers are quite buggy in general, there's at least a couple of known issues that if/when they get resolved might help with that too.

non survival difficulty, work ok, only survival mode, SS2 medic / doctor shop went bonker. really hate to run to DC just for heal my illness XD

I will give it another look. I keep forgetting which out of all the crafting recipes I have at chem/cooking stations are from other mods, so not everyone has as easy access to Antibiotics and such.

i look into agony as well, not sure it related, i grab patches already so i double check again :/

update: test vault 88 minutemen NPC assigned on SS2 medic station which worked. as intended

only SS2 and some workshop npc not working for some reason :/

tested on "Julia Powell and Paige Hunter" who has medical healing doctor dialogue which i try assigned them to SS2 medical store, but end up either trade or said this is not CHARITY (i have Caps sister!!! XD)
